Personal Plans
Services are legally required to keep up-to-date records of children in their care which must be reviewed on a 6-monthly basis

Out of school care services are legally required to keep up-to-date records of children in their care which must be reviewed on a 6-monthly basis (as a minimum) by parents/carers, as well as children. Personal plans should contain all necessary registration information such as name, DOB, contacts, any medical needs etc. as well as more general information about the child such as their likes, dislikes, skills they would like to gain etc. Services will often have registration forms and then 'All About Me' books for each child.

Through the personal plans services should be able to show that they are aware of each child's needs and how they are ensuring that these wellbeing needs are met.

SOSCN has sample personal and registration plans which services can access through our 'Wellbeing in Practice Training'.
